  1. Fill out the Scantron request form and be sure to provide a State account number for a journal transfer. The price is currently $.30 per form.
  2. Email the form to Donna Follett at Moon Library (dbfollet@esf.edu) and she will contact you to arrange delivery or pick up of your order.
